Provider Forum for Families
The Agency for Persons with Disabilities hosted a two-day residential provider forum on April 24 and 25 for families whose loved one will be transitioning out of Carlton Palms in Mount Dora. The families had individual meetings with an APD team to discuss the transition process, then they had the opportunity to visit with 26 group home providers interested in accepting individuals from Carlton Palms.
The Forum was held at Lake Receptions. Some families spent almost the entire two days talking to providers. APD wanted to meet face-to-face with families to answer all their questions to help them choose another provider to serve their loved one. About 60 families attended the Forum. Other families who weren’t able to travel held meetings via telephone. All families left the Forum with several residential options to narrow to their final selection.
